Facial Fat Transfer Recovery, Week by Week

Fat transfer recovery is easier than most facial surgery — but it has a psychological trap: you look over-treated before you look good, by design. Knowing the timeline defuses the panic.

Days 1–3: peak fullness

Your face is swollen well beyond the planned result — the deliberate over-correction plus surgical swelling stack together. Bruising appears, especially around the eyes if the tear trough was treated. The donor site (abdomen, flanks or thighs) aches like a hard workout. This is the worst-looking phase and it is entirely expected. Cold compresses only as instructed, sleep elevated, no pressure on the face, simple pain relief.

Days 4–7: turning the corner

Swelling begins its retreat; bruising shifts colour and starts fading. You're comfortable at home, walking daily, off pain medication or nearly so. Most international patients fly home in this window — puffy but presentable in sunglasses, with everything they need for the weeks ahead.

Weeks 2–3: socially fine

Most visible swelling and bruising resolve; concealer covers the rest. Many patients return to work around day 10–14. The face still reads fuller than the target — do not judge volume yet, and resist any urge to massage it down (pressure moves fat you want to keep).

Weeks 4–12: the quiet reshuffle

Now biology does the sorting: fat that established blood supply lives, the rest is gradually resorbed. Volume slowly refines from "slightly much" toward the intended result. Some asymmetry of settling along the way is normal — the two sides rarely deswell in sync. Donor-site firmness softens over the same period.

Months 3–6: the real result

Surviving fat is now stable, living tissue. This is the photo-comparison point, the "was it worth it" point, and — for the minority who want one — the sensible time to discuss a small touch-up. Skin-quality effects from nanofat, if used, are also emerging by now.

Rules that protect your result

  • No smoking — it directly reduces fat survival.
  • No facial massage, pressure or face-down sleeping in the early weeks.
  • Keep weight stable through the settling months.
  • Strenuous exercise off for ~2–3 weeks (per your surgeon), then rebuild gradually.
  • Judge nothing before month 3; decide nothing before month 6.
